At Trellis, we know that timely access to your primary care team is critical in optimizing one’s health. That’s why we offer same- and next-day immediate care appointments as well as scheduled primary care, physical therapy, behavioral therapy, and medication management appointments within a week.

We have flipped the healthcare experience, so you spend more time with your provider one on one. All of our appointments start on time, so you won’t be hanging around in a “waiting room”.

Easy appointment scheduling is great, but what happens when something goes wrong? Trellis members have a 24/7 hotline that connects them with their primary care team around the clock. Get the urgent advice you need from the people who know you best.

Trellis aims to meet people where they are. By utilizing modern technology, you can communicate with your Trellis care team directly by phone, text, email, or video visit to reduce the burden of in-person appointments.

Your care team includes a nurse practitioner or physician’s assistant leading your primary care with a physical therapist, licensed social worker, and a doctor of pharmacy to manage your whole health. This collaborative team is overseen by a Trellis physician to guide best practice and supported by a RN-led team of medical assistants for health coaching.

Trellis Health maintains all of its electronic health records in MyChart, so our members will have access to their medical history right on their phone. Even better, most hospitals use MyChart, which makes it even easier for all your providers to communicate seamlessly.

Trellis is an “Advanced Primary Care” (APC) practice that is shifting the way medicine is delivered by offering multiple preventive care services under one roof, provided by a collaborative team of providers. While traditional primary care feels reactive and fragmented, Trellis’ APC model leverages the expertise of primary care providers, physical therapists, psychotherapists, and pharmacists under one roof to create a cohesive plan of care for every unique member.

Michael Duffy
Chief Medical Officer
Michael Duffy, MD, is a respected clinical leader who has spent more than two decades leading superior teams to transform clinical systems. As Chief Medical Officer for Trellis Health, he is dedicated to transforming the challenges both patients and clinicians face in the traditional primary care model into a groundbreaking care system that benefits those working within it and served by it.
Mike’s career is marked by a commitment to patient-centered care, technological integration, and system-wide healthcare improvements. He has served as Chief Health Officer and EVP of Transformation at Senscio Systems, Inc., where he spearheaded the design of a virtual chronic disease management program, improved health outcomes for seniors, contracted with new provider groups, and doubled member engagement. As Senior Physician Executive at Northern Light Health Mercy Hospital, he led dramatic improvements to primary care practice, patient care quality, and staff satisfaction—including directing the institution's COVID-19 response.
Mike was also instrumental in shifting the hospital system’s population health strategy from fee-for-service to value-based reimbursement.
Mike holds an MD from Albany Medical College and a BS in Biology from Villanova University. He still ties his shoes with bunny ears, having never found a convincing argument to do otherwise. An oat milk latte with two extra shots helps fuel Mike’s game-changing ideas.

Allison Talon
Nurse Practitioner
Allison brings over 20 years of experience to her role as a Family Nurse Practitioner at Trellis Health, offering a unique combination of energy and empathy to deliver evidence-based care. Her patient-centered approach enhances outcomes and fosters a compassionate, supportive environment for both patients and colleagues.
Allison’s expertise spans a wide range of care, including sports-related injuries, minor orthopedic procedures, preventive medicine, menopause, and chronic disease management. Allison is known for her ability to collaborate effectively with multidisciplinary teams to develop comprehensive care plans, ensuring the best outcomes for patients. Allison also brings extensive experience in menopause, women's health, and urgent care to Trellis Health. She previously worked at Martin’s Point Health Care, where she provided a full spectrum of care, including family planning, chronic disease management, and preceptorship for Nurse Practitioners and medical students.
Allison holds a Master of Science degree in Nursing from Simmons Graduate School of Health Studies and certifications from the University of Massachusetts, Boston, in Sports Medicine, Adolescent Health, and Transgender Care. When not caring for patients, Allison enjoys honing her skills in photography, capturing special moments in sports, with pets, and at birthday parties. Allison’s favorite seasonally inspired coffees are iced chai oatmilk latte with sweet cream cold foam in the summer and a hot vanilla latte in the winter.

Tanja Schoenbrod, BSN, RN, CNOR
Registered Nurse
Tanja Schoenbrod is a registered nurse with extensive experience delivering patient-centered care, clinical excellence, and innovative process improvement for Mainers. At Trellis Health, she promotes optimal wellness by combining clinical expertise and patient advocacy, enhancing comprehensive care management of acute and chronic disease, and supporting members to achieve their health goals.
For 15 years, Tanja has been a leader in elevating patient outcomes across diverse clinical settings. Most recently, at Maine Medical Center, she led staff development initiatives—overseeing onboarding, regulatory education, and curriculum development for the nursing department. She has also served as a Nurse Educator at Southern Maine Medical Center, where she worked closely with interdisciplinary teams to establish best practices and successfully implement quality and safety protocols to improve patient well-being.
Tanja grew up in Arundel, Maine and earned her BS in nursing from the University of Southern Maine. Among many favorite activities, Tanja can often be found channeling her inner 12-year-old while sailing her Sunfish or savoring a hot latte with whole milk.

Keri Prince, Doctor of Physical Therapy
Physical Therapist
Keri Prince is a physical therapist specializing in orthopedic and sports rehabilitation. Passionate about using evidence-based, patient-centered care and interprofessional collaboration to help patients achieve their goals, she empowers Trellis Health members to regain their active lifestyles through a holistic approach to musculoskeletal care.
Keri has provided physical therapy services in a range of settings, from outpatient and surgical care to working as a travel physical therapist across various U.S. clinics and hospitals. Fluent in Spanish, she embraces cultural competence, ensuring that all patients feel understood and supported.
Keri holds a doctor of physical therapy degree from Duke University School of Medicine and a BA in Global Studies from the University of California, Santa Barbara. A former outdoor guide in Patagonia, Chile, Keri appreciates a good coffee—especially the cortado.

Morgan Konisky, LCSW
Licensed Clinical Social Worker
Morgan Konisky is a licensed clinical social worker with a deep commitment to holistic, patient-centered care. She believes physical, social, mental, and emotional health are all essential to a person’s well-being—and providing comprehensive support can make a life-changing difference. Morgan offers Trellis Health members mental health therapy, connects them with community resources, and collaborates closely with the care team to ensure an integrated approach to health.
For nearly a decade, Morgan has supported individuals managing chronic illnesses, substance use disorders, adjustment issues, grief, anxiety, depression, and acute mental health challenges in settings including in hospice care, psychiatric hospitals, skilled nursing facilities, and primary care clinics.
Morgan earned her BSW and MSW from Colorado State University. Running is her favorite, daily self-care practice. She’ll have a white chocolate mocha, thank you.

Evelyn Olson, Pharm.D.
Pharmacist
Evelyn Olson is a clinical pharmacist board certified in pharmacotherapy. Passionate about making healthcare more patient-centered, she leads Trellis Health initiatives that empower members to use medications safely and effectively—with a focus on balancing cost, accessibility, and the complexity of treatment regimens.
Evelyn brings over a decade of experience spanning primary care, outpatient, and hospital pharmacy settings to Trellis Health. She holds a BS in Biological Sciences from the University of Alaska Anchorage, a Pharm.D. from the University of Arizona, and an MBA from the University of Alaska Fairbanks.
A passionate cook, Evelyn has a love of most food—but don’t serve her black olives! Evelyn's beverage of choice is the Nushagak Fog, a lavender-infused twist on the classic London Fog, named after a river in her rural Alaskan hometown.

Jenelle Wing, NCMA
Lead Medical Assistant
Jenelle Wing is a nationally certified medical assistant who seamlessly balances clinical expertise, administrative efficiency, and empathetic leadership. She serves as a vital link between the Trellis Health care team and patients, ensuring all members feel listened to and comfortable as they receive high-quality care.
Jenelle brings eight years of service as a lead medical assistant, primarily in pediatric settings including Maine Medical Partners Specialty Care Pediatrics Gastro. In previous roles in health information management with Maine Medical center and in medical office management at Grace Street Recovery Services, clients benefited from Jenelle’s administrative expertise and interpersonal savvy in receiving the support they needed.
Jenelle grew up in Maine and earned her certificate in Medical Assisting from Kaplan University. In her free time, she makes park accurate Disney princess gowns and wigs—Rapunzel was her favorite—and attends comic conventions. Her beverage of choice is a dirty chai, extra hot with nutmeg.

Christine Bonica-Chase
Member Care and Recovery Specialist
Christine Chase, Member Care and Recovery Specialist for Trellis Health, is a seasoned Business Administrator with three decades of combined administration experience as a Regional Travel Manager consulting C-level executives with commercial and leisure travel accounts while simultaneously owning and operating a small business offering outdoor equipment and gear for active families. Prior to joining Trellis Health, Christine was part of York Hospital’s Care Access and Patient Registration teams in Southern Maine. As a Patient Care Representative and Senior Registrar, Christine was a vital member for communication and support staff across the Emergency Department, Lab, Cardiology, Family Practices and Wells Urgent Care by engaging as a bridge between patients and clinical providers.
Christine has a passion for operations management, offering exceptional customer service and exceeding expectations with sales and development. Additionally, she has an unwavering dedication to making every patient or client feel comfortable and understood as if they were a part of her own family. Outside of work, she volunteers with various Mental Health organizations like, Stay; For Life, a non-profit advocating for men’s mental health and suicide prevention, along with centers that support and provide care for anyone affected by or struggling with substance misuse and addiction to assist in educating and exploring multiple pathways to recovery. She has a warm vibe and uses her positive attitude and contagious energy to encourage others to work hard, aim high and succeed by integrating a team approach and building a healthy community!
Christine holds an A.S. in Business Management, Certificate in Travel & Tourism and is a Registered Recovery Coach. She loves being outdoors, creating new adventures and starts every morning with a continuous flow of hot tea with just a splash of light milk!

Amy Dugas
Director of Operations
Amy Dugas brings over a decade of experience in healthcare administration to her role as Director of Operations at Trellis Health. She is passionate about developing workflows and clinic operations that seamlessly envelop patients, elevating their experience and improving their health.
Amy excels in managing clinic and hospital operations through strategic oversight, budget management, and development of optimized workflows that ensure the delivery of quality care and exceed patient expectations. Before joining Trellis Health, she served as Regional Director for Primary and Specialty Care Services at Bridgton Hospital, a part of Central Maine Healthcare, where her leadership was instrumental in achieving regulatory compliance and high patient satisfaction scores.
Amy holds an MS in Organizational Leadership and a BS in Healthcare Administration from Southern Columbia University. She is a Certified Medical Practice Executive (CMPE) and has been recognized with a HEROES award (Healthcare Employees Reaching Out with Exceptional Service). A crafting guru who chooses to forgo the distraction of both internet and cable in her home, Amy drinks endless cups of coffee with a touch of cream.
